Required for CNA course:
l. Must be at least 16 years of age
2. Must have a valid State Identification card
3. Must have a Social Security Card
4. Physical Examination by a physician or nurse practitioner within 90 days prior to clinical rotation.
5. PPD (TB) test within 90 days prior to  our clinical rotation.
6. Must pass a Live Scan Fingerprinting background check (no cost to student)
7. The Live Scan Fingerprinting will be done on our mandatory orientation day at 6020 Commerce Blvd. #126, in Rohnert Park, CA 94928

Program tuition for North Bay CNA Training program is $1500:

North Bay CNA Training Program Student Uniform and Dress Code
To deliver the highest standards set forth by health and safety regulations:
Every student is responsible for maintaining the highest standards of dress, grooming, health and hygiene.
Please have the following, by the 1st day of class.  Uniform is not needed until the first day of clinical, (10 days after beginning date of class).


The following items are the responsibility of the student, with the exception of name badges:

Uniform:
White medical scrubs (to be worn on our first clinical day)
The student can wear nice street clothes during our theory days.

Shoes:
Any color, closed toe, closed heel with non-skid soles.

Watch:
Watch with a second hand. Easy removal of wristband.

Jewelry:
Our nurse assistant training program recommends no jewelry be worn. If you must wear jewelry, please make sure there are no dangling earrings or necklaces. Clinical facility protocols are to be followed regarding fingernails, and jewelry, which we will address on our mandatory orientation day.

Pen and Paper:
Ballpoint pens, black ink, highlighter, notepad.

Hygiene:
N
o perfumes or colognes may be worn in our CNA classes. Others may be sensitive to these. Always maintain good personal hygiene.

Hair:
Hair must be pulled back in a pony tail or bun in our clinical facilities and in our labs.

Name Badges:

Must be worn at all times in our CNA classes (name badges are provided by NBCNA).

Food and Beverages:
There is a 30 minute lunch break and two 15 minute breaks. Please provide your own lunch, snacks and beverages.
