To Register:
1. Fill out the 283B application form* (click here to download the enrollment application:  CDPH283bForm).
*Be sure to download and then save the form.  Then fill it out.

2. Email completed application form to cindyc@nbcna.com. You can bring the enrollment application in, on mandatory orientation day if not sent electronically

3. After completing the application form, please pay the $1500 tuition, which includes a $300 non-refundable deposit. You may pay by Check, Cash, credit card, or call, North Bay CNA Training Program, at 707.304.4131 to pay by credit card.
